Position Overview: The Licensed Social Worker (MSW or LCSW) is an essential member of the interdisciplinary hospice team, providing psychosocial support, counseling, and resource coordination for patients and their loved ones. Your Role: As a Part\-Time Licensed Social Worker (MSW or LCSW), you will: * Conduct psychosocial assessments and contribute to individualized care planning. * Provide emotional support, counseling, and crisis intervention. * Assist patients and families with advance care planning and decision\-making. * Connect families with community resources, including financial, placement, and supportive services. * Collaborate with nurses, physicians, chaplains, and the hospice care team. * Support bereavement and grief needs as appropriate. * Participate in hospice admissions and ongoing patient visits. * Uphold our mission that “no one should die alone.” Qualifications: * **Master’s Degree Social Work (MSW)** from an accredited program (required). * Prefers Licensed Clinical Social Worker * Current Social Work license in the State of Nevada (MSW or LCSW required per regulations). * Minimum of 5 years of experience, preferably in home health, hospice, or geriatric care. * Strong knowledge of community resources and healthcare systems. * Excellent communication and interpersonal skills. * Valid Driver’s License with an acceptable driving record. * Ability to travel within primarily Washoe County * Experience with electronic medical records preferred.
