**POSITION SUMMARY:**
The Spiritual Care Coordinator (SCC) provides comprehensive spiritual and emotional support services to patients, families, and caregivers in hospice and home health settings. The SCC is a core member of the Interdisciplinary Team (IDT), offering direct spiritual care and contributing to the holistic treatment plan. This position serves as a liaison with community clergy and spiritual counselors, ensuring the spiritual needs of patients and families are assessed and addressed with sensitivity, inclusivity, and professionalism.
**ESSENTIAL DUTIES AND RESPONSIBILITIES:**
*Including, but not limited to the following:*
* Conducts spiritual assessments for patients, families, and caregivers to identify spiritual and emotional needs related to illness, treatment, or life circumstances.
* Participates in developing and implementing individualized plans of care, integrating spiritual and emotional care into the overall treatment plan.
* Provides direct pastoral and spiritual counseling, emotional support, and crisis intervention to patients and families.
* Offers grief, loss, and bereavement support in collaboration with the social work and bereavement teams.
* Coordinates and collaborates with community clergy, chaplains, and spiritual care volunteers to ensure continuity and diversity of spiritual support.
* Facilitates funeral, memorial, and celebration\-of\-life services as requested by patients or families.
* Plans and leads periodic memorial services for staff, volunteers, and community members involved in hospice care.
* Provides education and consultation to staff and volunteers regarding spiritual and cultural aspects of end\-of\-life care.
* Maintains records of spiritual care visits and activities in compliance with agency documentation standards.
* Actively participates in interdisciplinary team meetings, contributing to holistic patient and family care.
* Assists in recruiting, orienting, and supervising spiritual care volunteers and community clergy partners.
* Participates in continuous quality improvement and performance evaluation activities.
* Maintains professional growth through ongoing education, peer supervision, and community involvement.
* Performs other duties as assigned.
**SUPERVISORY REQUIREMENTS:**
May provide supervision and mentorship to spiritual care volunteers and student chaplain interns as assigned.
**SUPERVISION RECEIVED:**
Supervisor will be the Director of Nursing
**KNOWLEDGE AND SKILLS NECESSARY TO PERFORM JOB RESPONSIBILITIES:**
* Ability to communicate effectively in English, both verbally and in writing.
* Competence in interfaith and multicultural settings; ability to respect and serve individuals from diverse religious, cultural, and personal backgrounds.
* Understanding of grief, loss, and end\-of\-life issues.
* Excellent listening, counseling, and interpersonal skills.
* Ability to collaborate effectively as part of an interdisciplinary team.
* Knowledge of hospice philosophy and spiritual dimensions of care.
* Computer literacy sufficient for documentation and communication tasks.
* Ability to manage multiple priorities and adapt to changing needs.
* Must be willing to travel throughout the agency’s geographic service area.
**QUALIFICATION REQUIREMENTS:**
*Education, Certification, and Licenses \-*
* Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree required).
* Certification or endorsement by a recognized faith group preferred.
* Board certification with the Association of Professional Chaplains (APC), NACC, NAJC, or equivalent preferred.
* Current Driver’s License and automobile insurance in accordance with agency requirements.
*Experience \-*
* Minimum of one year of experience providing spiritual care in a healthcare, hospice, or palliative care setting preferred.
* Experience with death, dying, grief, and bereavement required.
**PHYSICAL DEMANDS/REQUIREMENTS:**
* Regularly required to sit, stand, walk, and reach with hands and arms.
* May require bending, kneeling, and climbing stairs during patient visits.
* Ability to deal effectively with stress.
**WORK CONDITIONS/ENVIRONMENT:**
* Employee may encounter various environmental situations in individual patient homes, including odors, tobacco smoke or animals. Tasks may involve exposure to blood, body fluids or tissues.
* Travel is required within the entire service area.
* Employee may occasionally be required to work weekdays, holidays, or other hours on a rotation basis, as well as on\-call rotation.
