### **Qualifications:**

* Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings.
* Bachelor's degree strongly preferred.
* Leadership or management experience a major plus.
* Valid driver's license (required)

### **Responsibilities:**

* Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
* Ensure adherence to hospice regulations and standards.
* Facilitate communication between medical staff and the care team.
* Lead interdisciplinary meetings, documenting patient needs and care plans.
* Develop and update patient care policies and emergency procedures.
* Manage staff scheduling, performance reviews, and foster a positive work environment.
* Act as the central hub for communication with families, physicians, and other providers.
* Analyze quality metrics and manage a caseload of patients as needed.
* Participate in after-hours and on-call duties to ensure seamless care.
* Maximize resource utilization and promote a collaborative work culture.
